Pharmacy contractors set up as the parent organisation will have visibility of all their associated children within the main forms page of the Caforb portal. The Parent organisation should verify the accuracy of this information and, if amendments are needed, submit a request to the Welsh IG Toolkit Team via email on WelshIGToolkit@wales.nhs.uk, who will review and implement the required changes. Pharmacy contractors must clearly specify the parent entity and any additions or removals of children in their request.
