Informed verbal consent would be appropriate and whoever consents the patient should record it has been given on their system i.e. PMR or GP system, completion of a form is no longer required.

Responsibilities

Repeat Dispensing Data:  Contractors are able to find information on the level of repeat dispensing within their locality and health board areas.  This will help contractors benchmark their own performance.  The data can be accessed at at Repeat Dispensed – NHS Wales Shared Services Partnership
